If you're submitting a complaint on behalf of someone else, you must complete this form obtaining their signed consent and send it to IBAC.

Heads of departments and Council CEOs (among other ‘relevant principal officers’) must notify IBAC of any matter where they suspect on reasonable grounds that corrupt conduct has occurred or is occurring.
